In this position, you will help participants set life goals, navigate social situations, and develop essential life skills. You will work under the guidance of Clinical Case Managers, making a tangible difference in the lives of individuals facing various challenges.

Key Responsibilities:
• Document and maintain health care records for participants
• Write detailed incident reports in collaboration with case management




• Facilitate goal-setting processes for participants
• Maintain awareness of participant well-being
• Perform other related duties as needed

Requirements:
• Explicit criminal and abuse registry checks
• Experience supporting individuals with intellectual challenges
• Excellent communication skills
• Valid driver’s license preferred
• Available 15 – 25 hours per week
