As a Case Manager, you will provide critical support and holistic services tailored to the diverse needs of newcomers. Your responsibilities include conducting assessments, creating detailed settlement plans, and establishing trust-based relationships with clients. You'll coordinate essential resources, ensuring each client receives comprehensive support throughout their settlement process.
Key Responsibilities:
• Conduct detailed client needs assessments • Develop and adjust personalized settlement plans • Liaise with various partners to improve client access • Monitor client progress and provide ongoing support • Promote self-advocacy and independence among clients
Requirements: • Bachelor’s degree in Social Work or related field • Strong interpersonal communication skills required • Experience in trauma-informed care necessary • Ability to work both independently and collaboratively • Adequate computer skills in Microsoft Office and database use
