The Barrie & Community Family Health Team is looking for a 0.5 FTE Nurse Practitioner committed to delivering patient-centered primary health care. You’ll engage in health promotion, assess chronic diseases, and collaborate with a multidisciplinary team to optimize care. Strong advocacy for patient health and safety is essential.
Key Responsibilities:
• Deliver primary health care that meets CNO standards
• Implement health promotion and disease prevention strategies
• Improve patient access to diagnosis and treatment




• Consult with health care team members effectively
• Develop and coordinate individualized patient care plans
Requirements:
• Valid registration with the College of Nurses of Ontario
• NP-PHC program completion required
• Experience working in inter-skilled settings
• Strong problem-solving and communication skills
• Ability to manage electronic records and MS Office proficiently
