This PT RN Case Manager role will provide coverage in the Palliative Care, Medically Complex, Restorative and Behavioural Programs. In this leadership role the Case Manager is responsible for supporting admissions & discharges; coordinating the timely completion of the MDS Assessment Tool for a caseload of patients; ensuring continuity of care for a caseload of patients, which may include the provision of hands-on care such as ADL, transfers, bathing, wound management, management of IV & vascular access; managing patient & family concerns in a manner which supports patient focused care and problem-solving; promoting cooperative practice which includes coordinating care among team members; integrating the components of evidence based practice to improve the quality of patient care; contributing to program development including participation in Program CQI projects, managing resources in a fiscally responsible manner; and mentoring of nursing team members. The position reports directly to the Clinical Manager, Behavioural Health, for all functions pertaining to service operations and team functions on the assigned units. Graduate of an approved School of Nursing and currently registered with the CNO; BScN.
