The organization prioritizes excellence, collaboration, and community‑based programs.The Health Services Lead, Primary Health Care is a member of the Primary Health Care leadership team and is responsible for advancing priorities of the portfolio by supporting the development and implementation of assigned Primary Health Care, Wellness, Chronic Disease Management, and Department of Family Practice programs and initiatives. This role oversees day‑to‑day operations of multiple teams spanning wellness, chronic disease, and family practice collaborative teams. The Lead collaborates with other members of the Primary Health Care leadership team to implement projects that support patient‑centered, community‑based primary health care programs within their Zone.The Health Services Lead works closely with all interprofessional teams to manage performance standards and operational requirements by facilitating the development and implementation of consistent primary health care and organizational priorities, while respecting the uniqueness of practices and communities.Because the position requires travel within the Northern Zone, a valid driver’s license and reliable transportation are required.About YouBachelor’s degree in a health discipline or related field.Three to five years of recent,



progressive leadership experience in a health care setting.Experience in Primary Health Care is an asset.Current registration with the relevant professional association (if applicable).Demonstrated knowledge of primary health care, health promotion, determinants of health, and community health needs.Strong interpersonal and communication (oral, written, and presentation) skills, including collaboration with diverse professionals and community stakeholders, and a high level of judgment, tact, creativity, diplomacy, discretion, and confidentiality.Experience in program development, project management, implementation, monitoring, and evaluation in community settings.Ability to work with physicians and stakeholders across multiple governance structures to establish and sustain collaborative family practice teams.Experience in change management initiatives.Robust human resource and financial management skills.Physical and mental capability to perform duties such as working irregular hours and under pressure, maintaining multiple concurrent projects.Proficiency with computer applications.Experience in a unionized environment is an asset.Valid driver’s license and reliable vehicle.Competency in additional languages (French preferred).HoursLong assignment, full‑time position; 75 hours bi‑weekly.Approximately 15 months.Shifts may include evenings, weekends, and holidays.
