This contract position seeks a passionate individual dedicated to supporting youth facing homelessness. You will assess client needs, develop housing plans, and coordinate with local organizations. Your experience with marginalized individuals and a strong understanding of relevant legislation are essential for effective service delivery.
Key Responsibilities:
• Conduct intake interviews in diverse settings
• Initiate case management for street-involved clients
• Liaise with service providers for coordinated care




• Assist clients with housing applications
• Monitor and evaluate client needs regularly
Requirements:
• Degree in social or human services or equivalent experience
• Minimum two years of experience in a related field
• Skills in mental health and harm reduction practices
• Valid driver’s license and First Aid/CPR certificate
• Strong interpersonal and communication skills
