Join Carlington Community Health Centre as a Family Support Worker, focusing on assisting families and children aged 0–18. This role is integral to connecting vulnerable communities with essential resources and services.
This permanent position starts at 0.4 FTE with potential growth to 0.6 FTE, allowing you to impact families in need positively. You will deploy a trauma-informed approach to meet families’ diverse needs, whether related to food security, housing, or youth opportunities. Your proactive efforts in the community will foster equity and inclusion among diverse populations.
Key Responsibilities:
• Conduct needs assessments and develop family action plans
• Actively connect families to necessary community services
• Deliver programs designed to enhance family welfare
• Maintain relationships with community agencies for resource coordination
• Conduct regular home visits for current clients facing barriers
Requirements:
• Bachelor’s degree in Social Work or similar field
• 3-5 years working with vulnerable families
• Knowledge of community resources for children
• Fluent in English and French
• Capability to build trust with diverse families
Provide crucial support in navigating community resources at Carlington CHC.
