This position requires you to embrace a client-centered approach in a self-directed, team-oriented team. You will be expected to create tailored care plans that focus on holistic health — engaging with clients emotionally, socially, and spiritually. Key Responsibilities: • Facilitate interdisciplinary team care for clients • Support the implementation of comprehensive care plans • Participate in effective team communication practices • Perform delegated assessments and treatments • Identify potential safety improvements in client care
Requirements: • Diploma from an accredited OTA/PTA program • One year of relevant work experience preferred • Community care experience is an asset • Ability to communicate effectively in all contexts • Valid driver's license and transportation required
