The Social Worker II - Assisted Living Alberta plays a key role in supporting patient transitions and care coordination across Acute Care, Home and Community Care, and Continuing Care Home settings. This role provides clinical social work services with a robust focus on financial assessment, resource navigation, psychosocial evaluation, counseling, case management, and discharge planning. The ideal candidate demonstrates strong clinical judgment, excellent communication skills, and the ability to work both independently and as part of a collaborative healthcare team. This role requires adaptability, critical thinking, and a commitment to patient-centered care within a rapid-paced and evolving environment. This position primarily supports the town of Whitecourt, with the possibility of providing services to nearby communities as needed. Responsibilities span Acute Care, Home and Community Care, and Continuing Care Homes. Travel is required, and a reliable vehicle. Mileage or travel-related compensation is provided by the employer. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Certification in Basic Life Support - Healthcare Provider. Driver's License with vehicle suitable for North Zone travel 12 months of the year.

Connect Care training. Non-Violent Crisis Intervention certification. A minimum of one year of experience in Acute Care, Continuing Care, and/or Home Care. Enhanced knowledge of community-based housing supports and services. Experience and knowledge related to AHS/ALA system navigation, placement processes, transitions in care, and available financial supports.
