Trillium Health Partners in Mississauga is seeking a Nurse Practitioner to support the Regional Chronic Kidney Disease Program within Outpatient Renal services. The NP will provide advanced nephrology care, perform assessments, order tests, and coordinate care for CKD patients across Credit Valley Hospital, Renal Care Centre, and satellite sites.
