In this position, you will utilize your social work background (2-4 years) to engage with families and coordinate essential services. Your responsibilities include managing applications, attending case conferences, and ensuring that care meets the hospital’s vision. Key Responsibilities:
• Coordinate service applications for youth with special needs
• Attend and contribute to case resolution meetings
• Complete necessary complex special needs applications




• Oversee transition planning and placements
• Conduct required reporting according to hospital standards

Requirements:
• BSW from an accredited institution is mandatory
• Current certification from the Ontario College is required
• Experience in a children's social work capacity preferred
• Familiarity with local social services systems is advantageous
• French language skills considered a plus
