PCH seeks a proactive Social Worker to deliver essential psychosocial services. You will engage with residents and their families, employing a range of clinical skills to promote dignity and safety. This role critically supports vulnerable populations while ensuring compliance with relevant healthcare legislation and PCH standards.
Key Responsibilities:
• Complete comprehensive psychosocial assessments for residents
• Provide individual and group counseling for emotional support
• Develop and implement tailored social work plans
• Advocate for residents' rights and access to services
• Coordinate with interdisciplinary teams for optimal care
Requirements:
• Bachelor of Social Work from an accredited program
• Minimum three years in long-term care or related services
• Robust skills in crisis intervention and care planning
• Current registration with the Ontario Social Workers College
• Proficiency in electronic health records
