Support vulnerable populations as a Nurse Practitioner at Durham Community Health Centre with a focus on primary care and mental health. This temporary full time position allows you to make a direct impact in Oshawa and the surrounding area.
As part of the West Durham Interprofessional Primary Care Team, you'll deliver client-centered health care that promotes accessibility and addresses the needs of marginalized members of the community. Your expertise in mental health will be vital in performing assessments, guiding treatment plans, and collaborating with a diverse health care team.
Key Responsibilities:
• Provide primary care services for mental health concerns
• Execute clinical evaluations and therapeutic interventions
• Collaborate within an inter-professional health care model
• Manage client care and monitor progress
• Maintain thorough medical records using the EMR system
Requirements:
• Master's degree in nursing and registration as NP in Ontario
• Minimum of three years of experience in primary care
• Strong communication and problem-solving abilities
• Reliable vehicle and valid driver's license
• Full COVID-19 vaccination required
