Enhance lives as a Registered Practical Nurse with our Mobile Response Team in Brockville, Ontario. Deliver compassionate care for older adults with responsive behaviours related to dementia and mental health challenges.

This full-time position at Providence Care involves direct collaboration with RNs and PSWs to provide critical interventions in long-term care settings. You will assist in transitions and develop tailored care plans, leveraging your expertise in responsive behaviour management. Aiming for person-centred care, you’ll work with a dedicated team to improve the quality of life for clients.

Key Responsibilities:
• Provide direct care in collaboration with the MRT RN and PSWs
• Develop assignments and support MRT PSWs
• Collaborate with seniors/geriatric mental health teams




• Utilize evidence-based assessment tools for client evaluation
• Implement Cooperative Care Plans for responsive behaviours

Requirements:
• Current RPN registration with the College of Nurses of Ontario
• RPN Diploma with medication certificate
• Minimum one year related experience with older adults
• Experience in supportive management of responsive behaviours
• Strong written and oral communication skills
