Become a Geriatric Assessment Specialist at Bruyère Health, providing essential assessments for elderly patients. This full time, in-home role aims to maximize seniors' health and functionality.
You will serve on the Geriatric Assessment Outreach Team, working under the Manager of Outpatient Programs. This position requires conducting multidisciplinary assessments while engaging with caregivers and community services to promote independence. Your expertise will shape health recommendations tailored to client needs.
Key Responsibilities:
• Execute comprehensive in-home assessments for seniors
• Collect data for clinical evaluation purposes
• Facilitate independence through collaboration with resources
• Document assessments and recommendations clearly
• Promote health safety among elderly clients
Requirements:
• Bachelor’s in Nursing or Master’s in Therapy field
• At least five years of geriatric clinical experience
• Must possess a valid driver's license
• Fluent in both French and English
• Registration with Ontario's professional regulatory bodies
