You will serve on the Geriatric Assessment Outreach Team, working under the Manager of Outpatient Programs. Key Responsibilities:
• Execute comprehensive in-home assessments for seniors
• Collect data for clinical evaluation purposes
• Facilitate independence through collaboration with resources




• Document assessments and recommendations clearly
• Promote health safety among elderly clients

Requirements:
• Bachelor’s in Nursing or Master’s in Therapy field
• At least five years of geriatric clinical experience
• Must possess a valid driver's license
• Fluent in both French and English
• Registration with Ontario's professional regulatory bodies
