Become a pivotal C.A.R.E Connector at MyPotentia in Calgary, AB, where you’ll advocate for those impacted by Fetal Alcohol Spectrum Disorder (FASD). This full-time hybrid role emphasizes continuous, client-centered support for adults.
As a C.A.R.E Connector, you will work closely with clients and the Intake Team to develop and maintain personalized C.A.R.E plans. Your responsibilities will include navigating complex systems on behalf of clients, providing advocacy, and ensuring effective inter-agency collaboration to support comprehensive care.
Key Responsibilities:
• Create and regularly update personalized C.A.R.E plans
• Offer consistent support as per C.A.R.E directives
• Serve as a crucial advocate for clients in the community
• Coordinate multi-agency service delivery for optimal care
• Document all client care interactions and plans
Requirements:
• Diploma in Health, Social Services, or equivalent experience
• Familiarity with FASD and its community implications
• Solid interpersonal skills to build client relationships
• Detail-oriented and organized
• Must have a valid license and vehicle insurance
