AgeCare is seeking a dedicated RAI Coordinator to oversee MDS administration and implementation. This role focuses on training, monitoring, and supporting the interdisciplinary team to enhance resident assessments. You'll ensure accuracy and timely completion of MDS assessments while fostering a team-oriented environment.
Key Responsibilities:
• Coordinate and implement the Minimum Data Set (MDS)
• Provide MDS education and training to team members
• Monitor and support MDS documentation across the facility
• Maintain an up-to-date resident assessment schedule




• Ensure MDS compliance and coding accuracy for assessments
Requirements:
• Registered Nurse (RN) or Registered Practical Nurse (RPN) status
• Minimum three years of experience in Long-Term Care
• Knowledge of Long-Term Care Act and Ministry standards
• Strong organizational and leadership skills
• Commitment to quality care and resident well-being
