In this vital role, you will be integral to developing and implementing services that reflect the beliefs of the Métis Nation. Engage directly with clients through home visits, assessments, and community activities that foster wellness. Key Responsibilities:
• Assess and support client-driven needs effectively
• Conduct home visits and develop wellness plans
• Facilitate educational activities for community engagement
• Record and report services accurately in the OHATS
• Maintain confidentiality and adhere to privacy laws
Requirements:
• Completed degree/diploma in social services
• Minimum 1 year experience with client services
• Knowledge of Indigenous programs preferred
• Organizational skills that meet deadlines
• Ability to work independently and as part of a team
