Make a significant impact in healthcare as a Community Health Nurse with the Northwest Territories Health and Social Services Authority. This role focuses on patient-centered care, wellness promotion, and community health improvement.
As a key member of the Integrated Care Team, you will deliver primary care nursing and support health promotion activities. Your role will encompass assessing, planning, and evaluating patient care, while collaborating closely with interdisciplinary teams to coordinate comprehensive care.
Key Responsibilities:
• Deliver community health nursing services for wellness and prevention
• Assess and implement patient care for optimal health outcomes
• Coordinate care for patients with complex health needs
• Educate patients and families on health decisions
• Maintain accurate clinical documentation and support quality initiatives
Requirements:
• Nursing Degree with two years of recent experience
• Background in ER, Community Health Centre, or Public Health Unit
• Current registration with the College of Nurses of NWT and Nunavut
