Job summary

We are looking a dedicated and experienced Social Worker to join our dynamic team. As we are assisting many MVA clients, our Social Worker should be able to conduct a mental health assessment and establishing the report, coordinate care plans and, and advocate for the well-being of the clients. Your expertise in patient care, case management and provide support for clients are essential to navigate the complex challenges that the clients and their families experiencing.

Responsibilities

- Assess the social, emotional, and environmental needs of the MVA clients, develop personalized care plans and establish a comprehensive mental health report.
- Coordinate with clients and team members to schedule appointment.
- Provide counseling and emotional support to clients experiencing trauma or behavioral challenges from the MVA.
- Manage case documentation, including data collection, progress notes, and treatment plans, ensuring accuracy and confidentiality.




- Advocate for clients’ rights and connect families with necessary resources such as special education programs or community services.

Requirements

- Proven experience working in in healthcare or social service settings.
- Strong knowledge of patient care procedures, and case management principles.
- Ability to collect and analyze data effectively to monitor client progress and inform interventions.
- Excellent communication skills for collaborating with multidisciplinary teams and engaging with families empathetically.
- A valid license or certification as a licensed social worker (LSW) or equivalent is preferred.

Pay: $50.00 per hour
