About Us:The Niagara Falls Community Health Centre (NFCHC) is a non-profit, community-governed organization dedicated to providing accessible, comprehensive health services at no cost to residents of Niagara Falls. NFCHC offers a wide range of programs including primary care, dental services, mental health counseling, nutrition guidance, occupational therapy, and specialized support for 2SLGBTQ+ individuals, refugees, and newcomers. The Centre emphasizes health promotion and community development through free workshops and outreach initiatives designed to address social determinants of health. With a multidisciplinary team of healthcare professionals, NFCHC is committed to empowering individuals and fostering a healthier, more connected community.Main Responsibilities/Duties:Administer and document immunizations and injectionsDispense medication samplesPerform wound care, suture/staple removalConduct blood pressure checks and mental health follow-ups/check-ins via telephone or in-personApply Holter Monitors and home BP monitorsComplete school, public health, and employee forms (immunization forms)Perform phlebotomy and point-of-care testingCall patients with results and recommendations from MD/NPConsult with Niagara Region Public Health Vaccine Department for catch-up vaccine schedulesRequest urine drug screensManage vaccine fridge, ordering, and storage protocolsOversee medical and lab supply orderingMaintain accurate and timely EMR documentationAdhere to NFCHC policies, procedures, and health and safety legislationContribute to NFCHC’s strategic goals and uphold values of equity and diversityAdditional responsibilities as required under RPN Scope of PracticeEducation and Employment Requirements:Diploma in Practical Nursing from a recognized collegeRegistered and in good standing with the College of Nurses of Ontario (CNO)Current professional liability insuranceMinimum three years’ experience as an RPN in a community setting (primary care office experience is an asset)Demonstrated experience in collaboration and working within an interdisciplinary teamHighly proficient in Electronic Medical Records and use of virtual care toolsAbility to practice utilizing a client-centered approach with a strong understanding of the impact of social determinants of healthExcellent time management skills and ability to set prioritiesSuperior verbal and written communication skillsAbility to be flexible and adaptable to an evolving environmentStrong interpersonal skills and ability to positively collaborate with others and clientsProblem-solving skills and ability to collaborate towards solutionsCommitment to continual learning and professional growthCertified Diabetes Educator and/or Foot Care Certification would be an assetAdditional Requirements:Ability to work other hours as requested by leadership, including evenings and weekendsVisiting clients in various community settingsValid Ontario Driver’s License and access to a reliable vehiclePolice Reference Check requiredSome expectations of dissatisfied clientsExtended periods of sitting and working at a computerWhat we offerAnnual Salary: $48,038 - $56,175Health and Dental benefitsPension Enrollment (HOOPP)Professional Development Days + FundingPaid VacationShort-Term and Long-Term DisabilityApply Now!Please submit your resume and cover letter below.Thank you for your interest in joining our organization and for taking the time to apply.
