I feel welcome and at home.Pay RangeRate: $26.60 - $41.10 /visit (placement on salary range as per Collective Agreement)Estimated annual earning potential: $80,000–$130,000. Actual earnings may vary based on availability and service volumesKey Areas of AccountabilityAssess, monitor, and make care planning and care coordination.Evaluate care plan interventions through on-going reassessment and evaluation results.Ensure the quality of patient care by identifying and acting on factors which affect practice autonomy and delivery of quality care.Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.Support secure client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan,
and submit appropriate documentation to the case coordinator.Maintain documentation standards according to the College of Nurses of Ontario.Participate in team meetings, as required during the stated availability.Required QualificationsRegistered and in good standing with the College of Nurses of Ontario (CNO).Membership of your Skilled Association preferred (RNAO/WeRPN).RPN diploma/degree or RN degree from a recognised college or university.Have a valid Advanced Foot Care Certificate from a public-funded college or university.Completion of Advanced Foot Care Certification from Ruth Ruttan & Associates and Foot Canada Training are also accepted.Knowledge of the CAFCN – National Competencies for Advanced Nursing Foot Care in Canada guidelines is preferred.Worked a minimum of 100 hours as a Foot Care Nurse.Valid First Aid or CPR Certification.Knowledge of community nursing is an asset.Experience in foot care and gerontological nursing is an asset.Experience with Microsoft Teams and Microsoft Word is an asset.Demonstrate effective communication (written and verbal) and documentation skills.Demonstrate reliable attendance, punctuality, and availability for the role.Commitment to modelling lifelong learning and professional growth and can apply critical thinking skills to provide client care.Successful completion of the Physical Readiness Test (PRT) to ensure safe work practices are followed (transfers,
