The position requires you to perform comprehensive nursing assessments, support interdisciplinary teams, and advocate for clients' engagement in quality of life improvements.

Key Responsibilities:
• Implement nursing processes for patient assessments
• Develop and review individual care plans regularly
• Process physician’s orders and assist during visits




• Provide cognitive and emotional support to clients
• Participate in ongoing training for best practices

Requirements:
• Valid Licensed Practical Nurse License from CLHA
• Current CPR (BCLS) Certificate
• Proficiency in clinical assessment within scope
• Solid attention to detail and observation skills
• Experience in Long Term Care preferred
