Stonehenge Therapeutic Community seeks two Intensive Case Managers to work across Guelph-Wellington and Wellington County, primarily based in Mount Forest. You will conduct intake, assess needs, coordinate housing entitlements, and link clients to health and social services with partner agencies.
Some evenings and weekend rotation apply. Qualifications include a health/social services degree, at least one year in addictions/mental health, and solid documentation skills.
