Vancouver Island Health Authority is seeking a Community Outreach Intensive Case Management Nurse (RN/RPN) to support multi-site community settings in BC. The role focuses on comprehensive mental health and addictions services, coordinating care with clients, families and local health resources.

You will deliver client-centered rehabilitation plans using PSR principles, work with a multidisciplinary team, and travel between sites as required.
