In this energetic position, you will collaborate with clients to create and adjust individualized C.A.R.E plans, ensuring their ongoing needs are met effectively. Your efforts will focus on case management, including advocacy and coordination of services across multiple agencies while adhering to best documentation practices.
Key Responsibilities: - Collaborate on developing and updating C.A.R.E plans - Provide guidance and support outlined in client directives - Advocate within communities for clients with FASD - Coordinate services for continuous client care - Keep accurate records of client interactions
Requirements: - Diploma in Health, Social Services, or similar experience - Knowledge of FASD and its effects on communities - Excellent organizational and communication skills - Empathetic and nonjudgmental demeanor - Possession of driver’s license and insurance
