Key Responsibilities:
• Participate in community visits to address patient needs
• Deliver ethical and secure nursing care consistently
• Assess patients using effective techniques and tools
• Monitor and adjust care plans as necessary
• Build rapport with patients through collaboration
Requirements:
• Registration as a Registered Practical Nurse in good standing
• Valid driver's license and vehicle access required
• Experience in community care is an asset
• Reliability and flexibility for patient travel
• Willingness to provide on-call coverage as needed
