You will join an interdisciplinary team dedicated to delivering outreach and crisis intervention services. Your key responsibilities will include conducting thorough assessments, developing personalized care plans, and helping vulnerable clients navigate healthcare resources. Your expertise will be essential in reducing repeated acute service usage and facilitating transitions to better support.

Key Responsibilities:
• Perform mental and physical assessments for clients
• Deliver counseling to encourage healthcare connections




• Engage in crisis intervention for high-risk clients
• Partner with the team to create effective care strategies
• Help clients access necessary medical services

Requirements:
• Registered Nurse or Registered Psychiatric Nurse in BC
• Minimum 2 years of experience in mental health
• Valid BC Driver’s Licence and personal vehicle
• Solid knowledge of mental health and crisis intervention
• Excellent verbal and written communication skills
