Exciting opportunity for a Care Supervisor at CBI Home Health in Edmonton on a 12-month contract. Lead individual care management and uphold high standards in care delivery.
As a full time supervisor, you will coordinate assessments and monitor care plans for individuals receiving specialized community services. Your role includes establishing relationships with individuals and navigating stakeholder communications to ensure effective treatment. Take charge of clinical documentation and assist managers with oversight and reporting.
Key Responsibilities:
• Oversee clinical aspects of individual care
• Conduct intake and risk assessments with individuals
• Build trust and understand individual preferences
• Monitor and adjust care plans regularly
• Handle customer service inquiries effectively
Requirements:
• LPN, RPN, or other allied health education progress
• CPR Level C and First Aid certification
• Clear Criminal Record Check with Vulnerable Sector
• Valid Driver's License with vehicle access
• Minimum 1 year of experience with disabilities
