In this position, you will work closely with the Senior Manager to oversee Home and Community Care initiatives in line with community needs. Your responsibilities include managing staff, evaluating programs, and fostering effective communications with external agencies. An emphasis on inter-disciplinary teamwork is crucial to successful service delivery.

Key Responsibilities:
• Plan, implement, and evaluate Home and Community Care programs
• Liaise with external agencies to enhance services
• Ensure compliance with quality improvement standards




• Supervise daily operations and support staff trainings
• Review community needs to adapt service delivery

Requirements:
• Bachelor’s Degree or College Diploma in a health discipline
• Minimum three years’ experience in a health setting
• Proven leadership abilities in multidisciplinary environments
• Experience with palliative care practices
• Advanced skills in Microsoft Office Suite
