As a Diabetic Educator, you will implement interdisciplinary health maintenance plans for clients with diabetes. Key Responsibilities:
• Conduct comprehensive nursing assessments of diabetic clients
• Collaborate on health maintenance plans with healthcare professionals
• Use adult education methods to enhance self-management




• Maintain up-to-date medical records and coordinate with care providers
• Participate in outreach activities and diabetes education services

Requirements:
• Current registration with the College of Nurses of Ontario
• Minimum 2 years’ experience in a Diabetes Education Program
• Certification as a Diabetes Educator
• Experience with group and individual education facilitation
• Skill in utilizing EMR systems and software applications
