As a Connections Case Manager, you will work full-time until April 2027, helping young people aged 16-25 navigate housing instability while addressing barriers to well-being. You will focus on early intervention and collaboration across various organizations to ensure holistic support, guiding youth towards fulfilling their personal goals. This position requires flexible schedule, including evenings for program facilitation and early mornings for community outreach.
Key Responsibilities:
• Provide trauma-informed support to youth in crisis
• Assist in system navigation for housing stability
• Build trusted relationships to foster recovery
• Lead community outreach activities weekly
• Enhance natural supports for sustainable success
Requirements:
• Degree in Social Work or Child & Youth Care
• Minimum 2-3 years' experience with at-risk populations
