Join Flemingdon Health Centre as a part time Chronic Disease & Mental Health Service Worker. This role focuses on community outreach and chronic disease education, with expected evening and weekend hours.

In this part-time, 6-month contract position, you’ll engage with community partners and support various health programs. Your responsibilities will include outreach to healthcare providers, promoting diabetes education, and coordinating program evaluations. You'll collaborate within a multidisciplinary team to enhance community health and support service delivery across North York and surrounding areas.

Key Responsibilities:
• Liaise with community agencies to enhance chronic disease management
• Conduct outreach and promote education programs




• Support the delivery of health education sessions
• Organize data and submit evaluation reports
• Assist with volunteer training and engagement

Requirements:
• University degree or relevant experience
• Strong community outreach skills
• Ability to work with diverse populations
• Proficient in Microsoft Office and Canva
• Initiative-driven with strong organizational skills
