Become a Social Worker with Providence Care focused on access to care and transitions. This temporary full-time role utilizes your skills in psychosocial assessment and case management to improve patient recovery.
You will provide individual and family support, crisis intervention, and develop discharge plans while ensuring seamless transitions for patients. Advocacy and community linkages play a key role in connecting patients with essential resources and services.
Key Responsibilities:
• Conduct assessments and deliver supportive counseling
• Facilitate family and community support team conferences
• Develop and evaluate personalized care and discharge plans
• Coordinate with community agencies for effective transitions
• Keep up with treatment plan modifications as necessary
Requirements:
• Master of Social Work (MSW) required
• Registration with Ontario College of Social Workers needed
• Two years of relevant hospital experience preferred
• Experience with electronic patient records is advantageous
• Solid focus on patient advocacy and community collaboration
