You will join an interdisciplinary team dedicated to improving access and addressing the social determinants of health for residents facing barriers. Key Responsibilities:
• Provide comprehensive primary care within RN scope
• Coordinate care across the Health Centre and providers




• Deliver client education and health promotion
• Conduct telephone triage and follow up on results
• Support outreach programs addressing community needs

Requirements:
• Registered Nurse in valuable standing with College of Ontario
• Bachelor’s degree in nursing required
• Minimum three years’ experience in primary care
• Skilled in assessment, education, and counseling
• Experience in chronic disease management desirable
