In this full time, permanent role, you will work closely with clients facing homelessness and complex health challenges, including mental health and addiction issues. Your focus will be on delivering compassionate support while coordinating with various healthcare providers to meet clients' needs effectively.

Key Responsibilities:
• Engage with clients using a Determinants of Health framework
• Support the creation of personalized client care plans
• Deliver services in community settings and via mobile outreach
• Act as an advocate for clients seeking essential resources




• Participate in team meetings to enhance service delivery

Requirements:
• Relevant post-secondary education or social work background
• At least three years of experience in community development
• Familiarity with local health and social service systems
• Exceptional emotional intelligence and interpersonal skills
• Commitment to client-centered care and confidentiality
