In this role, you will report directly to the Manager of Resident Experience, focusing on both individual and family support within our Long-Term Care home. Your primary responsibility will be guiding residents through transitions while advocating for their needs. Your extensive knowledge and communication skills will be essential in enhancing the well-being and experiences of those in your care.
Key Responsibilities:
• Support residents throughout their collaborative living journey
• Assist families in advocating for their loved ones
• Promote dignity and quality of life for all residents
• Facilitate care conferences to ensure coordinated care
• Connect residents with relevant community services
Requirements:
• Accredited Social Service Worker diploma required
• Registration with the Ontario College of Social Workers
• Previous experience in long-term care preferred
• Fluency in French and English is mandatory
• Strong organizational and interpersonal skills
