Support the Reintegration Program team at St. Leonard’s Community Services as a Community Case Worker on an 18-month contract. Focus on community-referred individuals involved with the criminal justice system.

In this role, you will assess needs, manage daily supports, and create individualized plans for participants facing mental health and addiction issues. Solid advocacy and crisis management skills are essential. You will also connect clients with vital community resources and engage with various stakeholders.

Key Responsibilities:
• Conduct comprehensive assessments and case management
• Provide individualized support and discharge planning
• Network and build partnerships for client services




• Represent SLCS to ministry and police officials
• Work independently while multitasking in community settings

Requirements:
• Post-secondary education from an accredited institution
• Relevant experience of 1 - 3 years in social services
• Standard First Aid/CPR Level C certification
• Valid G Class Driver’s License and personal vehicle
• Bilingual candidates are highly regarded
