About the Role Our client is seeking a highly organized and dedicated Case Manager to join their remote social services team, serving clients across Northern Cape and nationally. In this role, you will be responsible for assessing client needs, developing comprehensive support plans, and connecting individuals and families with vital community resources and services. You will act as a key advocate, guiding clients through complex systems and empowering them to achieve their goals related to housing, employment, healthcare, and overall well-being. This position is ideal for a proactive professional experienced in remote client engagement and case management.
Key Responsibilities Conduct thorough remote assessments of client needs, challenges, and eligibility for services.
Develop and implement individualized case management plans in collaboration with clients.
Connect clients with appropriate resources, such as housing assistance, employment services, healthcare providers, and financial aid.
Monitor client progress towards goals, provide ongoing support, and adjust plans as necessary.
Maintain accurate, detailed,



and confidential client records using digital systems.
Advocate on behalf of clients to ensure they receive necessary support and services.
Requirements Bachelor's degree in Social Work, Human Services, or a related field.
Minimum of 2 years of experience in case management or social services.
Strong understanding of social service systems, community resources, and relevant legislation.
Excellent communication, organizational, and problem-solving skills.
Proficiency in using case management software and virtual communication tools.
Ability to work independently and manage a remote caseload effectively.
