Job Description The Heart Function Clinic Registered Nurse is responsible for performing assessments, developing a plan of care and implementing and evaluating the care of patients with Heart Failure in coordination with the Cardiologist and Pharmacist.
Performing clinical assessments of the new and returning patients to the clinic
Utilize the physical assessment and critical thinking in assessing patient symptoms or lifestyle changes in conjunction with heart failure guidelines and protocol to determine appropriate patient care plan
Coordinate the plan for patient care between the Cardiologist and Pharmacist assessments
Provide education to the patients regarding lifestyle and dietary adjustments associated with the heart failure conditions
Work collaboratively with the patient and the patient family to promote the appropriate condition management
Document the assessment information in the clinic specific information management systems according to the given guidelines for patient information management

Current registration in good standing with the College of Nurses of Ontario
Current BCLS-HCP certification required at time of hire
Current ACLS certification required at time of hire
Demonstrated clinical expertise in care of patients with Cardiovascular disease is required
Demonstrated knowledge of Heart Failure is required
Demonstrated knowledge of ECG interpretation is an asset
Demonstrated initiative and organizational skills
Demonstrated teaching skills
Experienced in verbal and communication skills
Satisfactory attendance and performance record
Physically able to perform the job
Additional Information Hours:
Monday - Friday, 7.5 hour day shifts (hours are subject to change based on operational needs)
Salary: $41.15 to $58.98 per hour
