Key Responsibilities:
• Support individuals with complex needs and system navigation
• Conduct comprehensive psycho-social assessments
• Collaborate with medical and community professionals




• Advocate for patient needs and social determinants of health
• Travel within Dufferin and Caledon for patient visits

Requirements:
• MSW with 2+ years or BSW with 4+ years experience
• Membership with OCSWSSW in good standing
• Proven experience in case management and navigation
• Knowledge of Dufferin-Caledon community health services
• Valid Ontario Driver’s License and access to a vehicle
