This permanent, full-time role entails 35 hours of work each week, with possible evenings and weekends. The Case Manager is pivotal in offering personalized care for clients aged 16 and older, integrating them into community resources and services to foster recovery and independence. Travel within York Region and South Simcoe is an essential part of this position.

Key Responsibilities:
• Maintain and support a client caseload
• Develop cooperative relationships with clients
• Create individualized goals and actionable plans




• Initiate referrals and provide short-term assistance
• Uphold client confidentiality and safety standards

Requirements:
• Degree in health or social services is preferred
• Full registration with a recognized regulatory body
• At least one year of relevant community experience
• A valid G driver's license with necessary insurance
• Knowledge of community resources in the area
