Key Responsibilities:
• Assess individualized patient care needs using assessment techniques
• Execute and monitor personalized care plans continuously

• Maintain and update qualified patient encounter records
• Provide on-call coverage after hours to ensure safety

Requirements:
• Registered Nurse with CNO in good standing
• Valid driver's license and access to a vehicle
• Community care experience is an asset
• Reliable, flexible, and willing to travel
