This is a temporary part-time position lasting six months, emphasizing teamwork in an interdisciplinary setting. Qualified candidates will have a BScN with a Nurse Practitioner certification and relevant experience in primary health care (2-5 years). Your expertise in chronic disease management and effective communication will be crucial in serving the diverse community.
Key Responsibilities:
• Provide comprehensive healthcare to clients in an interdisciplinary team

Requirements:
• BScN and Primary Health Care Nurse Practitioner qualifications

• Computer literacy with prior EMR experience preferred
