As a Nurse Practitioner with Access Alliance Multicultural Health and Community Services, you'll be part of a Primary Care Team focused on providing comprehensive health services. Your primary responsibilities will include diagnosing health issues, managing treatment plans, and collaborating with interdisciplinary teams to deliver exceptional care for underserved communities. The position involves working with diverse groups, ensuring all clients receive tailored, client-centered care.
Key Responsibilities:
• Perform assessments and manage treatment within NP scope
• Coordinate care and follow-up for diverse client needs
• Emphasize health promotion and disease prevention strategies
• Support community health initiatives in partnership with stakeholders
• Keep accurate records of client interactions and outcomes
Requirements:
• Master’s prepared RN (EC) with active Ontario registration
• Experience in primary care settings preferred
• Understanding of immigrant health challenges and needs
• Solid communication and problem-solving abilities
