Contribute to Indigenous community health as an Outreach Social Worker with Fraser Health. This role emphasizes harm reduction services and case management for marginalized clients in a healthcare setting.

Join Fraser Health’s Overdose Outreach Team as a Social Worker, providing critical support to Indigenous clients. In this position, you will engage in psychosocial assessments, coordinate care plans, and facilitate connections to resources. Candidates should possess a Bachelor’s degree in Social Work and experience within community-based harm reduction environments.

Key Responsibilities:
• Offer therapeutic and crisis intervention services
• Coordinate and manage individualized care plans
• Facilitate access to harm reduction programs




• Establish relationships with community health networks
• Maintain accurate documentation of client interactions

Requirements:
• Bachelor’s degree in Social Work and BC College registration
• Two years of experience in complex healthcare settings
• Experience with Indigenous populations and marginalized groups
• Valid B.C. driver’s license and access to a vehicle
• Current First Aid and CPR Level C certification

Assist Fraser Health in delivering essential support and culturally informed services aimed at improving client outcomes.
