Strong communication skills and the ability to collaborate with diverse teams are essential for success.
Key Responsibilities:
• Manage medications as part of a primary care team
• Conduct medication reconciliation and reviews
• Provide evidence-based drug information to healthcare providers
• Support patients in self-management and healthy behaviors
• Build care plans for chronic health conditions
Requirements:
• Bachelor’s degree in Pharmacy
• Licensed with Alberta College of Pharmacy
• Additional prescribing authorization within the first year
• Experience in trauma-informed care
• Ability to work flexible schedule when necessary
