Join WNHAC as a Homecare Nurse and provide essential health services within community settings. This role focuses on disease prevention and promoting client health through home visits.

In this full time position, the Homecare Nurse (HCN) will work with an interdisciplinary team to provide holistic health care. You will carry out assessments, collaborate with healthcare professionals, and support client health plans. Your expertise will enhance community wellness through preventative care programs and comprehensive client coordination.

Key Responsibilities:

- Conduct nursing assessments and treatments at home
- Work with team members to provide holistic care
- Develop and monitor customized client care plans
- Facilitate the client intake process and data entry
- Keep thorough medical records for accountability

Requirements:

- Registered Nurse with the Ontario College of Nurses
- Home care nursing experience is an asset
- Understanding of traditional Indigenous health practices
- Commitment to client-centered and de-colonial approaches
- Valid driver’s license and ability to travel
