Your OpportunityReporting to the program manager of NW Campus and working within the NW Adult community clinic (CAMHC‑NW), the RN/RPN Case Manager role is a team-oriented, person-centered strategy role for the provision of quality health and supportive services through the effective use of available resources in order to support the achievement of client’s goals.Within Addiction and Mental Health, the case managers support a recovery‑oriented approach and are encouraged to employ critical thinking to creatively plan and implement the most appropriate care in partnership with clients, families, and the multidisciplinary team as well as supervise and evaluate the care provided based on client health outcomes. The case manager also has the skill, expertise and capacity to take a leadership role in providing client‑and family centered care across the continuum of health‑care experiences and services. The RN/RPN’s scope of practice includes physical and mental status assessments, planning, coordinating, implementing and evaluating care as well as discharge planning for the purpose of promoting, maintaining or restoring mental health, and preventing mental illness, injury and disability. The RN/RPN provides care throughout the life span in all stages of health and complexities of illness. This position is responsible for communicating with other departments and services, across the Addiction and Mental Health continuum, to ensure the provision of comprehensive, high quality care to promote recovery and wellness.
