Human Services Navigator

1.0 FTE (35 hrs/week) until March 31, 2028

18 month contract with the possibility of extension

The Human Services Navigator plays a vital role supporting individuals who are at risk of, or are currently involved with, the criminal justice system. Working alongside justice, health, and social service partners, Navigators provide low-barrier system navigation, practical support, warm referrals, advocacy, and follow-up to address underlying needs and connect individuals with community-based supports. By improving access to services and resources, Navigators help individuals build stability, improve wellbeing, and reduce their risk of future involvement with the criminal justice system.

The ideal candidate is an innovative, collaborative, and creative thinker with a passion for community-based work and a strong understanding of the social determinants of health and the factors that can contribute to criminal justice system involvement. They will be comfortable working in a fast-paced, multidisciplinary environment with justice and social service partners. This person will be a self-starter with strong interpersonal and critical thinking skills, as well as the ability to build trust with folks navigating complex life circumstances.

Under the direction of the Community Wellbeing Lead, this position will:

- Provide system navigation, warm referrals, and support.
- Support individuals in identifying immediate needs and facilitate timely connections to appropriate onsite and community-based services.
- Work closely with partner organizations to support holistic, wraparound service planning that addresses underlying needs such as housing instability, poverty, mental health, substance use, trauma, family violence, employment, and basic needs.
- Establish and maintain strong working relationships with community partners.
- Maintain accurate and confidential client records,



ensuring compliance with relevant legal and ethical standards.

We are hiring for two Human Services Navigator positions within our Community Wellbeing team. While both positions share a common focus on system navigation, advocacy, and community-based support, responsibilities vary based on service location and need.

Qualifications

- A Bachelor of Social Work Degree or an undergraduate degree and equivalent experience
- Qualified designation with the OCSWSSW is considered an asset
- Working knowledge of relevant provincial legislation (e.g., Mental Health Act, Child and Family Services Act) pertaining to services provided by the agency
- Knowledge/experience working with evidence-based frameworks (e.g., CBT, DBT, Mindfulness) is considered an asset
- Experience working with, and extensive knowledge of, local community service agencies
- Demonstrated diplomacy and negotiation skills in client and system advocacy
- Excellent interpersonal, oral, and written communication skills
- Proven ability to work as a team member
- Demonstrated ability to exercise sound judgement and decision-making skills
- Proficiency with Microsoft Office programs, Internet research and database systems
- Ability to provide services in a second language (in addition to English) is an asset
- Valid driver’s licence and access to a reliable vehicle
